The gastric biopsy under direct vision by the fibergastroscope(1st Report)

Abstract
The roentgenographic, endoscopic and biopsies of the stomach are the most reliable methods to detect the various gastric diseases, especially early stomach cancers.
We have been using the gastric biopsy under direct vision by the fibergastroscope for the stomach lesion and recognized rapid increase of diagnostic accuracy for the stomach disease.
The authors have had an opportunity to study 50 cases of stomach disease who underwent the gastric biopsy under direct vision by the fiber gastroscope with type GF type B Olympus during the period from september 1971 to december 1971 at national medical center.
In a gastric biopsy under direct vision by the fibergastroscope of 50 cases, it was found that:
1) 4 cases (8%): normal, 5 cases(10%): gastric ulcer, 4 cases(8%): gastic cancer, 31 cases (62%): gastritis, 2 cases(4%): intestinal metaplasia, 1 cases(2%): intestinal metaplasia with atypism, 3 cases (5%): focal glandular epithelial hyperplasia.
2) The gastritis (31 cases) were consisted of follows: 21 cases (67.7%): superficial gastritis, 5 cases(16. 1%): atrophic gastritis, 1 case(3.2%): hypertrophic gastritis, 2 cases(6.4%): erosive gastritis, and 2 cases (6.4%) : nonspecific gastritis.
3) 7 cases of gastric cancer were diagnosed by the roentgenological examintion and 4 cases of them were confirmed with the correct diagnosis by the gastric biopsy.
4) Comparative positive findings among stomach x-ray, gastroendoscopic and gastric biopsy study was analiezed: The 18 cases of normal roentgenological findings were diagnosed with positive findings by the both endoscopic & gastric biopsy examination.
Among the 3 cases of stomach cancer, of which is confirmed by the stomch x-ray were found with 2 cases of gastric ulcer, 1 case of gastritis by the gastric biopsy.
1 case of the gastric ulcer showing by the stomach x-ray & endoscopic findings was shown to have gastritis by the gastric biopsy,
Judged by the fact present by above, it is concluded that those three methods combined with the roentgenologic, endoscopic and gastric biopsy examination increased the accuracy of diagnosis to almost over the 96%.
